Referral Forms

Open Doors Mental Health accepts referrals from a variety of sources, including youth, families, and professionals. Anyone may make a referral.

All services are voluntary and confidential. Submitting a referral does not obligate a child, youth, or family to participate in services.

​

If you are experiencing an emergency or are concerned about immediate safety, call 911 or visit your nearest hospital emergency department.

​

Please select the appropriate referral form below based on who is making the referral. Using the correct form helps ensure referrals are reviewed and processed efficiently.

RETURNING CLIENTS:

If you have accessed ODMH Services in the past, you can contact one of our Administrative Coordinators to book a session.​

WHAT TO EXPECT

When you reach out to ODMH, either by phone, email or online referral, one of our Administrative Coordinators will be in touch to book your first session.​

 

First contact will be through Single Session with a Clinician, either parent/caregiver only (if the child is under 12 years old) or with the youth. 

 

From there, services that fit your needs and the Clinician's assessment will be determined.
