Referral Forms

Open Doors Mental Health accepts referrals from a variety of sources, including youth, families, and professionals. Anyone may make a referral.

All services are voluntary and confidential. Submitting a referral does not obligate a child, youth, or family to participate in services.

If you are experiencing an emergency or are concerned about immediate safety, call 911 or visit your nearest hospital emergency department.

Please select the appropriate referral form below based on who is making the referral. Using the correct form helps ensure referrals are reviewed and processed efficiently.

If you have accessed ODMH Services in the past, you can contact one of our Administrative Coordinators to book a session.

Self Referral

This form is for youth who are seeking mental health support for themselves. Youth may complete this form on their own to request services from Open Doors Mental Health

WHAT TO EXPECT

When you reach out to ODMH, either by phone, email or online referral, one of our Administrative Coordinators will be in touch to book your first session.

First contact will be through Single Session with a Clinician, either parent/caregiver only (if the child is under 12 years old) or with the youth.

From there, services that fit your needs and the Clinician's assessment will be determined.
